New Client Forms/Treatment Instructions
New clients who are receiving the laser body sculpting treatments need to print, read, and fill out the two documents under the "Body Sculpting Services". New clients who are receiving any facial service should print, read, and fill out the forms that pertain to their treatment. However, every skin care client will need to fill out the "New Client Form: Skin Care" and bring it in to their first appointment.
